Patient Portal Registration
Step 1:
Click on the invitation link.
(If using phone/tablet: Click and hold invitation link to open it in internet browser)
Enter patient’s date of birth.
DOB verification is done to confirm that you are authorized to access this patient’s medical records.
If you get date of birth mismatch error, there is a possibility that an incorrect Date of Birth was entered upon registration. Contact us to correct this.
Step 2:
Enter your preferred 'Password' (follow the password instructions shown).
Fill in the CAPTCHA
Accept Charm PHR's privacy policy, terms, and conditions
Click on the 'Submit' button.
Enter the verification code received via email.
Click on the 'Verify OTP' button.
You will also receive a Welcome Mail from Charm with the PHR sign-in link for future use.
